Gynecomastia or enlarged male boobs is one of the most common cosmetic problems in young males. Due to this issue, young and middle-aged males are suffering socially and psychologically. They face cosmetic issues, loss of self-confidence, and low self-esteem. Males are greatly disturbed by gynecomastia and feel shy to remove their shirts in gatherings. They also feel uncomfortable when taking showers in pools or public places like beaches. Additionally, they feel discomfort when wearing short shirts and T-shirts, and their body image is disturbed by the movements of breast tissue when playing games like football, hockey, or others.
Males with gynecomastia often feel depressed and socially outcast in some cases. Due to this issue, gynecomastia surgery or enlarged male boobs surgery has become increasingly common, with many clients undergoing this procedure to eliminate their social dilemma. Over time, the size of the breasts increases, leading to greater discomfort.
Causes of Enlarged Breasts / Gynecomastia
- Idiopathic – Without any apparent reason
- Hormonal problems – Hormonal changes during puberty (17-18 years old)
- Medications – Side effects from certain drugs
- Use of steroids
- Bodybuilding and use of supplements
- Testicular problems
- Thyroid disease
- Renal or liver problems
- Obesity or increased weight
Grading of Gynecomastia
As the condition progresses, the size of the breasts increases, and the severity of the problem also escalates. According to the size, gynecomastia is divided into three main grades:
Grade 1
- Mild enlargement of the breast without skin laxity.
Grade 2A
- Moderate enlargement of the breast without skin laxity.
Grade 2B
- Moderate increase in size with looseness of the skin.
Grade 3
- Severe enlargement of the breast with loose, hanging skin.
Patients can experience different grades on both sides of the breast, depending on the size and skin laxity.
Assessment and Preparation
The patient or client visits Dr. Abdul Malik Mujahid’s clinic, where Dr. Abdul Malik himself conducts a thorough examination after taking a detailed history. During the assessment, Dr. Abdul Malik evaluates the size, shape, and symmetry of both breasts, the composition (whether there is more glandular tissue or fatty tissue), the position of the nipples and areola, and any chest or breast deformity.
Dr. Abdul Malik then discusses the patient’s concerns, explaining the procedure, surgery duration (which can be around one to two hours), post-operative care, recovery, expected outcomes, and potential complications. Baseline investigations are advised to ensure the patient’s fitness for anesthesia. The patient is also advised to follow the pre-operative instructions provided.
Procedure
Dr. Abdul Malik performs the procedure under either local anesthesia with sedation or general anesthesia, depending on the size and grading of the gynecomastia and the patient’s comfort level.
- Pre-operative markings are made, and the patient is draped in a sterile environment.
- A small incision of approximately 0.5 cm is made, and a special cannula is used for liposuction to remove excess fat.
- Following liposuction, an incision around the areola is made to remove breast gland tissue.
- After dissection, the gland tissue is removed, the wound is washed, hemostasis is secured, and the wound is closed with dressing.
Post-Operative Care and Recovery
After the procedure, the patient is shifted to the recovery room. Dr. Abdul Malik will visit the patient in the recovery room. If the surgery was done under local anesthesia, the patient is typically discharged on the same day. However, if general anesthesia was used, the patient may need to stay in the hospital for one day.
The patient is prescribed painkillers and antibiotics for one week. The following instructions are essential for recovery:
- Sleeping: Sleep straight to avoid strain on the surgical area.
- Compression Garment: Wear a compression garment for 4-6 weeks to help the skin retract and prevent loose skin.
- Swelling and Bruising: These can occur but typically resolve within 1-2 weeks.
- Altered Nipple Sensation: This is temporary and usually recovers in 3-4 weeks.
- Activity: Rest for 1-2 days and then start walking at home. Normal routine work can be resumed after 3-7 days. Gym activities should be avoided for 1-2 months for proper healing.
